Support for Independence(SFI) is seeking an experienced and motivated Care Coordinator to join our growing rehabilitation team. This is a key leadership role within the organisation, supporting the effective delivery of high-quality, person-centred support packages for our clients.

About the Role
As a Care Coordinator, youwill play a vital role in the day-to-day management and development of our Rehabilitation Support Worker (RSW) team. You will work closely with the Director, Case Managers and wider professionals to ensure support packages aredelivered safely, effectively and in line with CQC standards.
You will be responsible forover seeing staffing, rotas, assessments, supervision, training and compliance,while acting as a key point of contact for clients, support workers and professionals. This role requires strong leadership, excellent organisational skills and the ability to manage competing priorities in a fast-paced environment.
Key Responsibilities
· Provide leadershipand line management to Rehabilitation Support Workers
· Coordinate recruitment, induction, probation reviews, supervision and appraisals
· Liaise closely with Case Managers to ensure support packages meet client needs
· Plan and manage rotas, shifts and cover for sickness or unplanned absence
· Carry out direct observations of support staff and implement actions as required
· Prepare, reviewand update Support Plans, Risk Assessments and related documentation
· Respond today-to-day queries from clients and support workers
· Act as a firstpoint of contact during Case Manager leave (client specific)
· Investigate complaints and incidents in line with company policy
· Ensure confidentiality is maintained at all times
· Ensure SFI policies and procedures are followed consistently
· Ensure compliance with CQC standards, the Care Certificate and sector regulations
· Identify training needs and produce individual training plans for RSWs
· Deliver induction and mandatory training where appropriate
· Use feedback from clients and staff to support service development
· Support referrals and the development of new support packages
· Take a proactive role in networking to promote SFI’s support services
· Maintain accurate records and documentation on company systems
· Support theon-call rota system
· Work closely with the Registered Manager and Care Manager to ensure documentation is up todate
· Attend meetings as required and support ongoing organisational development
· Work flexibly to meet the needs of the service
Essential
· NVQ Level 3 in Health and Social Care (or equivalent)
· Experience in a supervisory or management role within the care sector
· Strong understanding of CQC standards and person-centred care
· Excellent communication, organisation and leadership skills
· Ability tomanage challenging situations calmly and professionally
· Ability to work independently and as part of a multidisciplinary team
· Full UK driving licence with business insurance and access to a car
· Flexible approach to working hours
· NVQ Level 4 or5 in Health and Social Care
· Knowledge or experience of acquired brain injury and complex needs
· Nursing, teaching or managerial qualification
· Strong IT skills (Microsoft Word, Excel, Outlook)
