We are seeking an experienced Associate Clinical Case Manager to work with Rehabilitation for Independence, supporting clients across the Northwest of England. This flexible associate role is ideal for an autonomous, clinically skilled professional who is passionate about delivering high-quality, person-centred rehabilitation to individuals with life-changing injuries and complex needs.
You will bring a professional background in Occupational Therapy, Physiotherapy, Psychology or Nursing, using your clinical expertise to coordinate rehabilitation, advocate for clients, and support meaningful outcomes. Working independently while remaining part of a supportive and collaborative organisation, you will play a key role in guiding clients through their rehabilitation journey.
About the Role
As an Associate Clinical Case Manager, you will manage a case load of clients with acquired brain injury, neurological conditions, sensory impairments, and other complex health needs. This is a varied and rewarding role, involving close collaboration with clients, families, solicitors, and multidisciplinary teams to deliver tailored rehabilitation programmes.
Your responsibilities will include holistic assessment, goalsetting, care coordination, monitoring progress, and adapting support as client needs evolve. You will act as a consistent point of contact, ensuring that rehabilitation plans remain client-focused, outcome-driven, and aligned with best clinical practice.
