Clinical Associate Case Manager

We’re looking for an Associate Case Manager who brings expertise and compassion in equal measure. If you have a background in Occupational Therapy, Physiotherapy, Psychology, or Nursing, this is your chance to make a real impact. You’llguide individuals through their rehabilitation journey, ensuring they feel supported, reassured, and in control of their progress.

What You'll Be Doing

·       Assess client needs and develop tailored rehabilitation plans

·       Coordinate and oversee therapy and support services

·       Work collaboratively with clients, families, and healthcare professionals

·       Monitor progress and adjust plans as needed to promote independence

·       Provide expert advice and advocacy to ensure clients receive the right support

·       Maintain accurate records and documentation in line with professional standards

About You

·       A qualified professional with a background in Occupational Therapy, Physiotherapy, Psychology, or Nursing

·       Experience working in neurorehabilitation, case management, or complex care (preferred but not essential)

·       Excellent communication and interpersonal skills, with a compassionate and client-centred approach

·       Strong organisational skills and the ability to manage a diverse caseload

·       A proactive problem-solver who can work independently and as part of a team

·       Commitment to professional development and delivering high-quality care
